Confused sentence

"The Toolkit provides clinicians with the ability to document clinical observations for their patients with chronic conditions and use the Toolkit to prompt them for tests and appointments that are due depending upon the condition they have been diagnosed with."

The final "they" does not refer to the patients but to the clinicians which is not what the writer intended.

Rewrite as "Clinicians use the Toolkit to document clinical observations of their patients' chronic conditions. They also use the Toolkit to prompt them about appropriate tests and appointments for each patient."

Fearing that directive settings could be misused by third parties (e.g.: insurance companies, telemarketers, etc) the system has been designed to limit and (in some cases) prevent directive lookups.

"Fearing" must be connected to noun or pronoun. The author declares that the system was fearing misuse of directive settings.

My rewrite:
"The system has been designed to limit and prevent direct lookups because directive settings could be misused by third parties."

In the context of HL7 Vocabulary the term ‘Realm’, it is used to indicate a Binding Realm: that manages the bindings/substitutions of value sets to reflect local rules, and is a parameterization permitting internationalization of vocabulary – binding different value sets to the same message design being used in a different country .

It's not really clear what is being conveyed here. What does it mean for something to have become increasingly fragile? I could understand this if the systems had become fragile, but I'm not sure what it means that they have become increasingly fragile.

A simpler version will do:
MEIA’s business systems are inflexible, fragile,and difficult to adapt to
legislative changes
